As an approved provider of Home Care Services as part of the Commonwealth Home Care Packages program, Clayton Church Homes can help you to maintain the independence you enjoy in your own home for as long as you wish.

Am I eligible for Home Care?

If you are finding daily tasks more difficult and need some help or support in order to remain living independently at home, you are likely to be eligible to access Clayton Church Homes’ services under the Australian Government My Aged Care scheme.

If you are not eligible for a government-subsidised Home Care Package, Clayton Church Homes can provide the required care and support on a fee for service basis.

We will guide you through the process of applying for a Home Care Package or work with you to tailor a combination of services to suit your needs.

What is a Home Care Package?

A Home Care Package is a range of services that make up a ‘package’ of care, which is designed to support you to maintain your independence at home.

Your needs are identified and confirmed with a case manager from My Aged Care, who will then work with you and your carers and/or family to create a care plan detailing the services you will receive. This care plan can be reviewed and changed at any time.

What is My Aged Care?

My Aged Care is the Commonwealth Government gateway for all aged care needs, including assessment and information on aged care support and programs across Australia.
