Bethanie and HBF Partner Up For Community Health Support Program

Bethanie, Western Australia’s largest not-for-profit aged care and retirement living provider, has partnered with HBF - one of WA’s biggest health service providers, to offer a unique community based Health Support Program for members. 

Following a hospital stay, eligible members from HBF are invited to participate in the Health Support Program for either a 3 to 6 month period. They are provided with support from a qualified Bethanie health coach in the comfort of their own homes.

The designated health coaches attend home visits, conduct phone support, and undertake reviews of the member’s current health status.

Bethanie provide the Health Coaches working on the project, who come from a range of clinical backgrounds. All Health Coaches are currently Registered Nurses or Allied Health Professionals, including physiotherapists, occupational therapists and dietitians.

The Coaches also liaise with members’ General Practitioners to ensure that the very best health care co-ordination is offered, with relevant medical histories taken into consideration.

Bethanie’s HBF Health Support Program Team Leader, Melanie Baker, tells, “The Health Support Program is a great opportunity to assist members who have long term health conditions to remain as independent as possible. The program aims to be a partnership between the member and health coach for a self-care approach to health and wellbeing”.

“The partnership between HBF and Bethanie has been very rewarding and we are thrilled to be able to team up together for such a fantastic and beneficial program. We commenced in March this year and are now seeing great health results that members are achieving, after being the first graduates from the 6 month program.”

Graham Stacey participated in the Health Support Program after experiencing health conditions, which made some of the smallest chores very difficult for him. Graham and wife Wendy are delighted with the program, as Graeme is now exercising daily and has improved both his fitness and general wellbeing.”

Karen Lambie, Graham’s Health Coach, comments, “The program is so unique in the sense that it focuses around the member’s individual health goals and we look at the priorities that members are most concerned about.”

Bethanie provides a range of other community based care services to support people in their homes including community care, as well as providing retirement villages and residential aged care facilities.
