There are two ways that you can access home care services.

You may be able to receive Government funding for a Home Care Package. To find out if you are eligible, you will need to be assessed by your local ACAT – Aged Care Assessment Team.

Once you have been assessed and received approval for a Home Care Package, you will be advised of your home care level. These levels are determined based on your individual care needs:

• Level 2: Low-level care
• Level 3: Intermediate care
• Level 4: High-level care

Each Home Care Package level receives different amounts of funding from the Government. Once you have been approved and received your Home Care Package funds, you will be able to choose the home care provider that suits you. Home care providers in Queensland include:

A new model of Consumer Directed Care (CDC) was recently introduced to home care. This means you now have even more flexibility and choice in selecting your home care provider. You can now also select the exact home care services you want and choose how your home care funds are spent.

If you are not eligible to receive Home Care Package funding from the Government, you can also self-fund your own home care program.
