Life, Made Easier with Care Connect

Meet Leo, who lives independently at home with help from Care Connect. Since the passing of his wife, Mike and his daughter Lilly make regular visits to check in on him. Leo enjoys social outings, keeping active and time being “Pop” to Lilly.

Challenges faced:

Leo is challenged with ongoing back issues that regularly impact his mobility. He now lives on his own and misses the company of his wife and the way they shared chores and responsibilities, which helped them manage their independence together.

Without a Care Manager:

Without the expertise and local knowledge of his Care Manager, Leo may begin to feel overwhelmed, especially with reduced mobility. Leo may also miss out on services and supports he is eligible to access through his Home Care Package.

The Care Manager difference:

• Maya is a passionate and helpful Care Manager who provided the expertise and support Leo needed. Mike and Lilly are relieved and now visit "Pop" to enjoy some good social times like they used to.
• With a family of his own and a full-time job, Mike reached out to Care Connect when he realised, he could not provide all the support his dad needed to live alone.
• Maya met in person with Leo and Mike to ask questions and to understand the challenges and what Leo enjoyed.
• Maya is dedicated to making Leo’s life easier, arranging personal care, cleaning, and weekly hydrotherapy. All of which contributed to Leo’s overall health and well-being.
• To empower Leo to live safely and securely at home, Maya has also arranged a personal alarm for him to wear.
