Uniting is one of the largest and oldest not-for-profit aged care providers in NSW and the ACT, with a long history of supporting seniors to stay safe and independent in their own homes and communities. Our Home and Community Care team is committed to helping people continue to live life in the ways they choose, with high-quality services tailored to each client’s unique needs, circumstances, and budget.

Building a close home care relationship means that we can proactively support clients through their changing needs as they grow older, giving them continuity of care and peace of mind.

Clients may start off receiving domestic assistance like cleaning or meal preparation, then down the track they may require nursing support or care for more complex needs. Knowing our clients well helps us to give them the best support at the right time.

All in the neighbourhood

It’s not surprising that our holistic approach to each client’s care helps to establish trust from the start, and continues to build on that trust over time. We liken our team of carers and coordinators to a ‘neighbourhood’ that’s focused on quality service delivery. ‘Circle of support’ meetings with the neighbourhood team and a client’s GP, clinical professionals, and invited family members, further ensures that everyone is fully supported at every stage of their care.

Staying connected

Uniting’s Family Portal app also makes it easy for everyone to stay in touch, and across, the home care services that are scheduled for each client. This is especially comforting for loved ones who may live far away.

The granddaughter of Dulcie, a long-term Uniting home care client who lives in Tamworth, shares the peace of mind her mother experiences from using Family Portal: “Living in Port Macquarie, Mum really appreciates the app – as it helps her feel more included and involved with Nan’s daily routine.
