Home Care Packages

Our experienced Home Care Packages Care Management team considers your individual situation and matches you with the care services you need.

We offer:

• Competitive Care Management rates:

We don’t charge basic daily fees.

An Income Tested Fee may apply for some clients, which is determined by an income assessment by Services Australia.

• Care Managers are available:

9am to 5pm Monday to Friday.

• Personalised and tailored integrated approach:

This starts with assessment, care planning and monitoring to ensure you are getting the care and support to meet your needs

• Experienced Care Management team:

All of our Care Managers are tertiary educated and are highly experienced in their field of work.

• Unique combination of private and publicly funded health services

We offer a broad range of services to meet physical, mental and social needs

• Choice and flexibility in the care people receive at home, as their needs change
• Strong Advocacy - our Care Managers can advocate for you to ensure your care needs are met  

In Home and Community Support Services

As an approved provider of a wide range of health services and supports, we empower you to have a choice and decide how and when our tailored services are delivered. From assistance with everyday tasks like showering and taking medication, to transport and connecting with the local community, you can choose from our services and determine what level of assistance you need to live comfortably and independently.

• Social Support Groups
• In Community Assist
• Domestic assistance
• Day respite care
• Home maintenance
• Gardening
• Community Access (transport)
• Meal preparation
• Personal care

Some of the services offered, such as Home Maintenance and Gardening require an Occupational Therapy assessment and approval from Care Managers
