Our Case Managers are all experienced health professionals with tertiary qualifications in nursing and/or allied health (some bi-lingual). This ensures we have the knowledge and skills to assist with all of your ongoing support issues and health needs
We are passionate about consumer directed care. A personalised and flexible package of support services is designed with you, specifically based on your identified needs.
We are responsive. Your calls will be personally answered and promptly responded to. This is due to our extensive team of case managers, administrative staff, two dedicated clinical coordinators, program manager, on-call case management systems and after hours support.
We represent value for money. You can select your preferred level of case management involvement and corresponding budget allocation. We do not charge a daily care fee or deduct interpreter costs from your individual package funds.
Our connection with Northern Health enables you timely access to additional health services and interpreters of all nationalities and languages.
