Rembrandt Living - Home Care

We know that remaining in your own home is very appealing but that at times you might benefit from some extra support. Rembrandt can provide you with that additional help allowing you to live with confidence and ease using either long or short-term services.

Depending on your level of care as determined by your ACAT (Aged Care Assessment Team), there are two types of home care services and assessments:

• Entry Level Services or
• Home Care Packages

Our Entry-Level Home Care Services, funded by the Commonwealth Home Support Program (CHSP), provide basic support services to assist you to remain independent at home.

For those with more complex needs, Home Care Packages provide specialised in-home support with a wide range of services that will assist you to remain living in your own home.

Potential services include:

• Domestic, gardening & home modifications
• Food preparation and delivery
• Shopping & transport
• In-home respite
• Personal care
• Social support including group classes
