SwanCare offers two separate home care service categories - Home Care and Home Care Plus.

Home Care is SwanCare’s core home care service designed to support the Australian Government’s Home Care Package program:

• Government funded
• ACAT assessment required
• Goal orientated
• Long term

Home Care Plus is a flexible add-on service:

• Periodic or short-term care
• Can be used as an add-on service to complement your Home Care Package
• NO ACAT required
• Fee-for-service (independent from the Government-funded program)
• No age restrictions
