HammondCare At Home offers home care services in Melbourne.

When you nominate HammondCare At Home as your preferred home care provider, we will get in contact with you to organise a visit in your home.

The range of care and services available under a Home Care Package includes, but is not limited to:

• Assistance with daily self-care tasks such as bathing, dressing, grooming and taking medication
• Transport and personal assistance to help you get out and about i.e. appointments or groceries etc.
• Allied health support including Podiatry and Physiotherapy
• Nursing care and other clinical services
• Continence management
• Mobility and dexterity support to remain mobile
• Help with domestic tasks such as general house cleaning, unaccompanied shopping and linen services.

If you require more extensive care than is specified in your government-subsidised Home Care Package, we can offer you additional Private Care services until My Aged Care is able to refer you to a higher-level Home Care Package.
