Aged Care Online

Uniting AgeWell

130 Lonsdale Street, Melbourne, Victoria, 3000

Uniting AgeWell believes that older people, like people of all ages, want to live in an environment of choice, empowerment and wellness – and to access support and care as they choose.

Research indicates that people stay healthier when they are in control of their environment and can maintain community connections. Uniting AgeWell strives to:

  • concentrate on what people can do, and seeks to connect people to their passions
  • focus on supporting choice, independence and doing-with, rather than doing-for
  • promote wellness for all residents and clients, wherever they are on their life journey
  • pay careful attention to the spiritual and pastoral needs of clients, as well as their physical, intellectual, social and emotional needs
  • give support and high quality clinical care when needed

Uniting AgeWell extends and connects all existing aged care services. It follows the biological ageing journey and gives the client choice, autonomy, flexibility, and community.

Uniting AgeWell's care model has six components which focus on well-being and the promotion of independence:

  • AgeWell Connect: Connecting people to the whole of their community
  • AgeWell Being: Supporting wellbeing of body, mind and soul
  • AgeWell Assist: Assisting people with services to live at home
  • AgeWell Restore: Restoring wellbeing through therapy services
  • AgeWell Care: Providing respite and residential care
  • AgeWell Palliate: Providing support and spiritual guidance at the end of life

This care model is designed to be flexible, enabling the recipient to determine what services they need and where those services will be delivered.
